MY HEALTH ONLINE APPOINTMENTS & CORONAVIRUS
As per advice received, we have temporarily stopped access to patients booking appointments online. It is essential that all patients who are invited to book an appointment are screened by reception to ensure we are able to identify people who may prove a risk; having travelled recently to the affected parts of the world. If you require an appointment please contact the surgery.
If you feel that you may be affected DO NOT ATTEND THE SURGERY, contact us via the telephone and remain in your home.
Request Online Services
My Health Online is a new online service brought to you from NHS Wales. My Health Online offers patients the convenience to book appointments using the internet, although this option has been, suspend now, you are still able to order repeat prescriptions online.
If you wish to register to use My Health Online, you must be over 16 and will need to complete the following form. To protect patient confidentiality you must provide documents so we can make sure you are who you say you are. Due to the current circumstances, we do not want patients bringing this proof to the surgery and we ask that you please send us a photo of your ID or if possible scan the relevant ID to our email address: email@example.com
Forms of identification need to be photo ID (e.g. driving licence or passport). If you do not have the facility to send a photo or scan documents, please call reception where a set of questions will be asked to verify who you are.
Once your form is received and your identity has been verified, we will process your request and forward you, via email your activation key and a set of instructions on what to do next.
