Me and My Health
If you have a long-term physical or mental health condition, this scheme aims to help you and your carer to provide clear information to health and care staff who may need to visit your home in an emergency.
Smoking
If you are a smoker, we strongly advise that you stop.

This service provides patients with:
- An initial assessment to determine the client’s readiness to stop smoking.
- Motivational interviewing techniques to assist clients in their attempt to stop smoking.
- Weekly supply and support in use of Nicotine Replacement Therapy products to maximise therapeutic benefits.
- Weekly Carbon monoxide monitoring to support a stop smoking attempt.
- Identify clients who need referring, following the local referral pathway.
What you can do to eat well
You know your eating habits better than anyone else. Have they changed? What do you think you can do to improve them? Eating well can help us keep strong, healthy and optimistic and with some planning, make our money go further.

Weight management service
We offer a range of services for adults in the Aneurin Bevan Health Board area who would like support with losing weight. These services will provide you with the knowledge, tools and support you need for successful, sustainable weight loss with a focus on promoting health and wellbeing.
During the pandemic some of our services will be delivered remotely by telephone or video link. More details about the services provided and how you can refer yourself can be found below.
Children under 16 years can be referred to 'Connect' Child and Young Person Weight Management Service.
How to access support to lose weight
You can refer yourself for support to lose weight. You can do this by either filling out an online referral form or by telephone, where a member of our team will complete the form with you.
